The short answer
Two actions in the same category, such as two purchase actions, both recorded conversions recently. That may be two real forms or one result counted twice. Oppy prints both numbers and the gap to the primary one, and asks someone who knows the site to settle it.
Why it matters
Unasked, an account can report a few hundred sales or several times that, depending on which purchase action is primary. Every cost per sale and ROAS in the account hangs on that choice.
A page view counted twice is noise in a number nobody bids on. A sale counted twice moves every figure in the account, and a lead counted twice moves what the business thinks a lead costs.
How Oppy decides
The check reads every conversion action in the categories that cost money when counted twice: purchases, lead forms, call leads, sign-ups, bookings and paid subscriptions.
Checks that weigh money or clicks judge what they find on your own account's numbers, not on one figure applied to every account, so what counts as a problem on a small account is not the same on a large one. Checks on settings read how the account is set up.
When there is too little to go on, the check says so instead of guessing. A finding always comes with the rows behind it, so you can read the evidence before you decide.

Check it yourself in Google Ads
- In Google Ads, open Goals, then Conversions, then Summary, for the last 30 days.
- Group the actions by category and note any category with two that both recorded conversions.
- Compare each one's count with your shop or CRM for the same days.
- Check which one is Primary.
What to do about it
Keep the action whose count matches your own records as Primary, and set the other to Secondary.
If both are real, for example two different forms, rename them clearly so the next reader does not think they overlap.
